Apply for Short-term Disability	Complete a Leave of Absence Form When directed by Human Resources, call the number on Cigna's How to Apply for Short-term Disability to initiate a Short-term Disability Claim Forms - When instructed by HR, have your doctor complete one of the HealthCare Provider Certificate Documents (see below) for FMLA to run concurrent with STD. Once the Doctor has completed the form, it should be returned to Human Resources directly.
Apply for and Process Military Leave Employee Handbook pages 4-10 and 4-11	Leave for military or Reserve duty shall be granted to full-time and part-time regular employees. If you are called to active military duty or to Reserve or National Guard training, or if you volunteer for the same, you must submit copies of your military orders to your supervisor as soon as it is practicable. During your annual training period of up to a maximum of fifteen (15) days, the Town of Milford shall compensate you only for the difference between your annual training pay from the military and your normal weekly (less payroll deductions) pay from the Town. Such military leave shall not be charged against sick or annual leave. Should an employee desire to return from temporary or extended military service, he/she shall be offered re-employment in his/her former position consistent with applicable State and/or Federal laws. Step 1 - Employee - Provide your military orders to you supervisor as soon as possible. Step 2 - Employee - Complete a Leave of Absence Form (LOA) and provide to your Supervisor. Supervisor signs and then the Town Administrator signs. Step 3 - Supervisor/Dept Head - Forwards a copy of the leave orders and the request for military leave (LOA) form to Human Resources and cc: Payroll Step 4 - If the employee is taking the leave without pay, they will keep the check issued by the government. Step 5 - If the employee is requesting to be paid, the Town only pays the difference between your annual training pay from the military and your normal weekly (less payroll deductions) pay from the Town. This is for a maximum of 15 days (120 hours) annually. The check you receive from the government should be signed over to the Town (provided to Finance). Finance will issue a check back to the employee once Finance processes the leave. The LOA form and military orders will be retained in the employee's personnel file. Step 6 - Employee or Supervisor/Dept Head - On the time sheet, in the comment section, please specify this is Military Leave of Absence.

Intiate Retirement Paperwork?	When an employee decides to retire from employment, he/she should first make an appointment with the NHRS three (3) months prior to the anticipated retirement date to get his retirement payments in order to be effective the 1st of the month following date of retirement. The employee should forward a letter to his/her department head and HR of his/her intent to retire. If the retiring employee is under 65 years of age, then he/she can continue being covered under the Town’s health insurance with the cost for the premium deducted from his/her retirement check (the Town does not bill for a retired employee – the premium must come from the retirement check). If a retiring employee is 65 or above, he/she must have Medicare parts A & B and he/she is eligible to enroll in the Town’s Medicare Supplement plan (currently Colonial Penn for health insurance and Humana for prescription coverage). If the retiring employee is interested in enrolling in the Town’s Medicare Supplement plan for the first of the month following date of retirement, he/she will need to contact Gary Daniels of Bankers Life (office – 603.224.5384). If you, as the retiring employee, elect to continue dental coverage, HR will notify the dental carrier that you are now in retiree status. Payment will be due quarterly, in advance, to the Town of Milford, Finance department. (Ex: If you are retiring at any point during a quarter, payment will be due for the remainder of that quarter (Jan-Mar; Apr-Jun; Jul-Sept; Oct-Dec). Both you and the Chair or Department Head have to sign the affidavit Once HR receives the Volunteer Affidavit, we can send the letter to HH (cc'ing the volunteer) Then once you have signed up with Hampshire Hills, call HR to make an appointment to complete the Town's paperwork You will need to sign two forms with Human Resources - An Agreement with the Town of Milford A Promissory Note What if I want to change my membership to an individual or a family plan? Contact Hampshire Hills regarding the change. Once the change is made, Hampshire Hills should inform the Town of the change, but you should reach out to the Town as well so I can prepare a revised agreement and/or deduction authorization (for employees). Then I can either email you the agreement (you sign and return) or you can make an appt and come in to sign the agreement. IMPORTANT: The Town can not alter your arrangements with Hampshire Hills, you have signed a contract with Hampshire Hills. The Town can not terminate your membership with Hampshire Hills, you have signed a contract with Hampshire Hills. If you leave the Town of Milford's employ, you will no longer be eligible to receive the Town discount.
