HOW DO I....

Fill out my W-4

 

The Town cannot recommend how to fill out your W-4 because we a) are not tax professionals, b) do not have access to information about other household income or second jobs, and c) don't have access to prior gross earnings for the year which can impact what the withholding should be.  We can recommend a) speaking with your tax advisor, b) utilizing the link to the IRS Withholding Calculator and/or c) monitoring your withholding (and assessing as your earnings grow) to determine if the appropriate amount of tax is being withheld.

 

For your benefit, we are providing links to the following YouTube informational videos which may be of assistance. These links are for reference only and are not endorsed by the Town.  The Town is not recommending this resource as a sole basis for determining how to complete your W4.  It is one of many resources that are available to you on the internet.  This is not a substitute for speaking with your own tax advisor. You can search on-line "how do I fill out the IRS Form W4 2023" yourself and find additional resources.

What are the steps Employees have to do to sign up for Hampshire Hills?

  1. Contact Finance (Cindy Lundberg - clundberg@milford.nh.gov or 603-249-0640) for a letter verifying you are an employee of the Town of Milford. (this letter can be emailed to HH or you can pick up in Finance).
  2. Once you have signed up with Hampshire Hills, call Finance to make an appointment to complete the Town's paperwork
    1. You will need to sign forms with Finance -
Employees (20 hrs or more)​​​​​​A Promissory Note and Agreement with the Town of Milford
 A Payroll Deduction Authorization
  
  1. For regular full-time (RFT = 40 hrs/wk) and regular part-time employees (20+ hrs/wk), your memberships fees can be deducted from your bi-weekly payroll.
  2. For those working LESS than 20 hrs/wk (call firefighters, limited part-time, per diems, etc), you will be billed quarterly, with payment due in advance of the quarter.

What are the steps Volunteers, Employees under 20 hrs/wk, call, per diem, etc.) have to do to sign up for Hampshire Hills?

  1. Ask your Chairperson or Department Head to complete a Volunteer Affidavit attesting that you volunteer at least 6 hours per month.  Both you and the Chair or Department Head have to sign the affidavit
  2. Once Finance receives the Volunteer Affidavit, we can send the letter to HH (cc'ing the volunteer)
  3. Then once you have signed up with Hampshire Hills, call Finance (603-249-0640) to make an appointment to complete the Town's paperwork
    1. You will need to sign forms with Finance - 
Employees (under 20 hrs/wk or Volunteers​​​​​​A Promissory Note and Agreement with the Town of Milford
  

What if I want to change my membership to an individual or a family plan?

  1. Contact Hampshire Hills regarding the change.
  2. Once the change is made, Hampshire Hills should inform the Town of the change, but you should reach out to the Town as well so I can prepare a revised agreement and/or deduction authorization (for employees).
  3. Then Finance can either email you the agreement (you sign and return) or you can make an appt and come in to sign the agreement.

IMPORTANT:

 

  1. The Town can not alter your arrangements with Hampshire Hills, you have signed a contract with Hampshire Hills.  
  2. The Town can not terminate your membership with Hampshire Hills, you have signed a contract with Hampshire Hills.
  3. If you leave the Town of Milford's employ (or stop volunteering), you will no longer be eligible to receive the Town discount.
  4. If you go from a full-time to limited part-time, the billing will no longer be through payroll deduction, but will change over to quarterly billing.

How do I make payments?

Payments can be -

  • Mailed to:  Finance, 1 Union Square, Milford NH  03055
  • Dropped off:  to anyone in Finance or Human Resources
  • Use the Tax Drop box in the vestibule 24/7
Initiate Family and Medical Leave (FMLA)

The Town Milford automatically runs sick leave or worker's compensation leave and FMLA concurrently. (Chapter 5. C)  Intermittent FMLA will occur when a doctor's note dictates the need for such leave.

Intiate Retirement Paperwork?
  1. When an employee decides to retire from employment, he/she should first make an appointment with the NHRS three (3) months prior to the anticipated retirement date to get his retirement payments in order to be effective the 1st of the month following date of retirement.
  2. The employee should forward a letter to his/her department head and HR of his/her intent to retire.
  3. If the retiring employee is under 65 years of age, then he/she can continue being covered under the Town’s health insurance with the cost for the premium deducted from his/her retirement check (the Town does not bill for a retired employee – the premium must come from the retirement check). 
  4. If a retiring employee is 65 or above, he/she must have Medicare parts A & B and he/she is eligible to enroll in the Town’s Medicare Supplement plan (currently Colonial Penn for health insurance and Humana for prescription coverage).  If the retiring employee is interested in enrolling in the Town’s Medicare Supplement plan for the first of the month following date of retirement, he/she will need to contact Gary Daniels of Bankers Life (office – 603.224.5384).
  5. If you, as the retiring employee, elect to continue dental coverage, HR will notify the dental carrier that you are now in retiree status.  Payment will be due quarterly, in advance, to the Town of Milford, Finance department.  (Ex:  If you are retiring at any point during a quarter, payment will be due for the remainder of that quarter (Jan-Mar; Apr-Jun; Jul-Sept; Oct-Dec).  If you retire in April – your dental premium would be deducted from your April paycheck and you would owe for May & June.  Kathy Townsend, Finance, would then bill quarterly going forward.)
  6. STD/LTD insurance(s) are to be cancelled when an employee retires.
  7. Life Insurance - You will be provided with information to convert or port your existing life insurance.
